Post-Acute Care Physician – Cortland, NY

A growing national physician group is looking for a post-acute care physician to join its Cortland, New York team. This full-time role combines rounding in skilled nursing facilities four to five days per week with medical directorship duties, blending hands-on patient care with clinical leadership.

Responsibilities

• Perform detailed patient evaluations, including medical histories and physical examinations

• Build, carry out, and update personalized care plans as needs change

• Follow patient progress and coordinate care to improve results

• Team up with physicians, nurses, and other health professionals

• Guide patients and families on managing health and preventing illness

• Keep precise, prompt medical records that comply with regulations

• Provide medical directorship and clinical leadership at assigned facilities

Schedule

• Full-time, four to five days per week rounding in skilled nursing facilities, plus medical directorship responsibilities

Compensation

• ~$230,000-$260,000 annually, fee for service, with no cap on productivity-based earnings. Medical directorship includes an additional monthly stipend.

Qualifications

• Active New York medical license and DEA registration

• Prior work in post-acute, hospital, emergency, or clinic environments is a plus but not essential; recent graduates are encouraged to apply
