Post-Acute Care Physician – Fort Morgan, CO

Maverick.MD is recruiting for a growing physician group seeking a full-time physician for its post-acute care team in Fort Morgan, Colorado.

You'll round five days per week in skilled nursing facilities, delivering continuity of care to a diverse patient population.

What you'll do

• Perform comprehensive patient assessments, including histories and physical exams
• Develop and adjust individualized care plans based on patient needs
• Coordinate with nursing staff, advanced practice clinicians, and facility teams
• Manage admissions, discharges, and transitions of care
• Maintain timely, accurate clinical documentation

Compensation

• $225,000-$275,000+ (fee for service, no cap on productivity)

Qualifications

• MD or DO with an active Colorado medical license and DEA registration
• Primary care background (family medicine or internal medicine)
• Strong communication skills and a team-oriented approach
• No unresolved disciplinary issues
